This card may be printed out if you would like to be an organ and tissue donor in the case of your death. However, it is essential that your family knows your wishes since they may be asked to sign a consent form to authorize your donation.

To learn how donated organs are distributed in your area, use the contact information provided in our state directory to locate your local organ procurement organization (OPO).
