While still in medical school, William Palmer (Keith Allen) vows to become the most famous surgeon in England. Brilliant, handsome, and confident, the discriminating doctor seems on the fast track to success. He soon marries the loving and supportive Annie Thornton (Jayne Ashbourne), who vows to do "whatever it takes" to help him succeed. For a time, that means denying that William could be involved in a string of deaths that hit terribly close to home.

Palmer cannot escape debt, but continues to elude suspicion. Still, his desperation is becoming impossible to hide. When racing partner John Parsons Cook (Richard Coyle) takes ill at the track and later dies, Cook's stepfather (Stephen Moore), suspicious of the too-solicitous Palmer, demands the unheard of: an autopsy.
