Special conference focuses on improving major health care transitions
On Tuesday, nurses, aides, doctors and other staffers discussed communication problems that plague the health care system during a special conference, called “Mind the Gap.” Gaps occur when patients go from the ICU to rehab or go home after surgery — all without the proper information. Dr. Jessie Israel of Complete Care organized the gathering in Holmdel to help close patient information gaps.
