A damning federal report found multiple failures in infection control at Andover Subacute Rehab II where 17 bodies piled up in a makeshift morgue over Easter weekend. The report fined the facility $220,000 for lapses, including failure to notify families and physicians when patients' conditions worsened during a COVID-19 outbreak.
