While lawmakers spar over the future of health care, pilot projects on the ground are focusing on how to improve the lives of low-income patients. Special correspondent Jackie Judd reports on one such program in Montana, where health care players team up to identify high-needs patients and go well beyond the traditional bounds of medical care to prevent routine visits to the emergency room.
